    1. Completed new patient forms

    2. Your current insurance card

    3. Physician referral forms (if required by your insurance carrier)

    4. Any past records, including blood tests, past allergy testing, recent imaging or other relevant documents.

  • On your first visit, Dr. Shepherd will review your history, focusing on whatever your main concerns are. This is followed by a physical exam and discussion of testing and treatment.  You can expect to leave with a detailed treatment plan that will improve your symptoms.

  • Skin testing for allergies can be performed, but it is typically planned for a follow up visit once your symptoms are under better control and appropriate medications can be stopped prior to testing.  Not every patient requires skin testing, so we do not ask patients to stop medications prior to their initial visit. 

    Pulmonary function testing is typically performed as well if you have respiratory issues. Patch testing can be performed if you have problems with skin allergies and contact dermatitis.  

  • If indicated, Dr. Shepherd may ask you to have blood labs drawn or ask to see lab work that you previously had drawn elsewhere.  Try to bring this with you to the visit if you have access to it.
